Healthy Klamath’s Extended Network For Diabetes

The prevalence of diabetes among adults in Oregon has more than doubled — an increase of 124% — over the past 20 years. There are approximately 287,000 adults with diagnosed diabetes in Oregon and an estimated 110,000 adults with diabetes who do not know it.* Chances are you've come across many patients with prediabetes or diabetes. We're here to help.

That’s why Healthy Klamath has created an extended network for diabetes. With funding from the Health Resources and Services Administration, we are working to create a network of programs, resources and partners to help prevent and manage diabetes in our community.

    Here is the OMA’s toolkit with resources that help to identify and refer patients with prediabetes to National Diabetes Prevention Programs (National DPPs), recognized by the Centers for Disease Control and Prevention (CDC).
